JOB SUMMARY: Utilizes internal and external resources to assist older adults to function at an optimum level of independence through comprehensive health assessment, care planning, linkage with essential healthcare services and monitoring of service delivery outcomes.



  • Knows and supports the Catholic Charities of Buffalo NY mission, vision and guiding principles;
  • Assists in providing a comprehensive, individualized, strength based, culturally responsive medical and mental health assessment to identify resources that increase participation and support patient-centered goals;
  • Works with the inter-disciplinary team, consisting of care coordinators, other professionals, caregivers and family members, to comprehensively assess clients' functional abilities (ADL, IADL, PAM), progress and barriers to achieving/maintaining wellness and independence;
  • Conducts initial, periodic and crisis health assessments;
  • Reviews, monitors and informs clients regarding medications including effects and interactions;
  • Educates clients in health and wellness topics, including but not limited to, falls prevention and disease self-management;
  • Refers and links clients to healthcare services and entitlements (e.g. MLTC);
  • Maintains clear and concise documentation of interventions that demonstrates compliance with the principles of interdisciplinary teamwork as well as funder requirements and accreditation standards;
  • Facilitates continuity and coordination of care by sending/obtaining client health records to/from primary and specialty providers;
  • Co-signs discharge summaries;
  • Provides consultation to the social day program for the purpose of planning and facilitating structured, comprehensive health and wellness programming for participants;
  • Utilizes data to identify areas of needed improvement and implements improvement plans in support of achieving performance targets, program goals, client satisfaction and positive client outcomes;
  • Adheres to the Agency's compliance policies, code of ethics, laws, regulations, and ethical standards.
  • Links with client's primary care provider to obtain diagnosis and consultation about physical needs.



  • Establishes and maintains cooperative and collaborative relationships with co­ workers/professionals within Catholic Charities and in peer organizations and professional associations;
  • Maintains current license and meets all requirements for continuing education.


ACCOUNTABILITIES/SUPERVISORY RESPONSIBILITIES: Works under the direct supervision of the Program Supervisor and in cooperation with the interdisciplinary team.




EDUCATION: Licensed Registered Nurse (RN) preferred. Candidates with a Licensed Practical Nurse (LPN) license plus substantial experience providing direct assessment and care to mental health impaired older adults may qualify at the discretion of the agency.


EXPERIENCE: Three (3) years experience working directly with mental health impaired and senior populations.



  • Basic understanding of psychiatric and substance use disorders in older adults;
  • Willingness to work within an inter-disciplinary team approach;
  • Excellent interpersonal and team building skills.
  • Willingness to provide services in the community throughout the City of Buffalo and suburbs;
  • NYS driver license and maintenance of reliable personal transportation.
